Bitter melon (Momordica charantia) is a tropical vine whose fruit, seeds, and leaves have been used for centuries in Ayurvedic, Chinese, and African traditional medicine, primarily for blood sugar management. The fruit contains several bioactive compounds with hypoglycemic activity: charantin (a steroidal saponin), polypeptide-p (an insulin-like peptide), and vicine. These compounds work through multiple mechanisms including activation of AMP-activated protein kinase (AMPK), enhancement of glucose uptake in skeletal muscle and adipose tissue, stimulation of pancreatic beta-cell insulin secretion, and inhibition of intestinal glucose absorption. Clinical trials in patients with type 2 diabetes have shown modest reductions in fasting blood glucose and HbA1c levels, though results are heterogeneous. Bitter melon also contains antioxidant compounds including phenolic acids, flavonoids, and vitamin C. It is available as fresh fruit, juice, capsules, and powdered extract, though the strong bitter taste limits palatability of whole-fruit preparations.
Multiple clinical trials show that bitter melon extract reduces fasting blood glucose levels in patients with type 2 diabetes through AMPK activation, enhanced glucose uptake, and improved insulin sensitivity.
