Boswellia serrata, commonly known as Indian frankincense, is a tree native to India whose oleo-gum resin has been used in Ayurvedic medicine for millennia. The primary bioactive compounds are pentacyclic triterpenic acids called boswellic acids, of which acetyl-11-keto-β-boswellic acid (AKBA) is the most pharmacologically potent. AKBA is a selective, non-redox inhibitor of 5-lipoxygenase (5-LOX), the enzyme responsible for leukotriene synthesis, a key driver of inflammatory cascades. Unlike NSAIDs, boswellia does not inhibit cyclooxygenase enzymes, which spares the gastrointestinal tract from common NSAID-related side effects. Clinical trials have demonstrated significant improvements in pain, stiffness, and physical function in osteoarthritis patients, as well as potential benefits in inflammatory bowel disease (ulcerative colitis), asthma, and rheumatoid arthritis. Modern standardized extracts such as 5-Loxin and AprèsFlex offer enhanced bioavailability of AKBA.
