D-Mannose is a simple sugar related to glucose that is used primarily for the prevention and management of urinary tract infections (UTIs). It works by preventing E. coli bacteria from adhering to the walls of the urinary tract, facilitating their excretion in urine.
D-Mannose is a naturally occurring monosaccharide (C₆H₁₂O₆) that is structurally similar to glucose but is only minimally metabolized by the body, with the majority excreted unchanged in the urine. This property makes it particularly useful for urinary tract health, as D-mannose saturates mannose-specific receptors (FimH adhesins) on type 1 fimbriae of uropathogenic Escherichia coli, competitively inhibiting bacterial adhesion to uroepithelial cells. A pivotal randomized controlled trial demonstrated that 2 g of D-mannose daily was as effective as nitrofurantoin in preventing recurrent UTIs, with significantly fewer side effects. Additional clinical studies support its role as a prophylactic agent in women with recurrent UTIs. D-mannose does not significantly affect blood glucose levels despite being a sugar, as approximately 90% is excreted renally without metabolism. It is generally well tolerated, with mild gastrointestinal symptoms as the most common adverse effect.
A randomized controlled trial showed that 2 g D-mannose daily reduced UTI recurrence rates comparably to the antibiotic nitrofurantoin, with significantly fewer adverse effects over a 6-month period.
D-mannose binds to FimH adhesins on E. coli fimbriae, preventing bacterial attachment to uroepithelial mannose receptors and allowing bacteria to be flushed out during urination.
Once daily, dissolved in water
For acute UTI symptoms, some protocols use 1.5–2 g every 2–3 hours for the first 48 hours, then reduce to twice daily. This does not replace antibiotics for confirmed infections.
