Devil's claw is a Southern African plant traditionally used for pain and inflammation. Its primary active compound, harpagoside, has demonstrated anti-inflammatory and analgesic effects in clinical trials for osteoarthritis and low back pain.
Devil's claw (Harpagophytum procumbens) is a perennial plant native to the Kalahari Desert in Southern Africa, where its tuberous secondary roots have been used for centuries by indigenous San peoples for musculoskeletal pain and digestive complaints. The primary bioactive compounds are iridoid glycosides, particularly harpagoside, which inhibit COX-2 and pro-inflammatory cytokines. Several randomized controlled trials in Europe have demonstrated that devil's claw extract is effective for reducing pain in osteoarthritis of the knee and hip and in chronic low back pain, with efficacy comparable to low-dose NSAIDs in some studies. It is approved as a traditional herbal medicine by the European Medicines Agency (EMA).
Clinical trials show that devil's claw extract (containing ≥50 mg harpagoside daily) significantly reduces pain scores in hip and knee osteoarthritis compared to placebo.
A four-week RCT found that high-dose devil's claw (60 mg harpagoside daily) was non-inferior to rofecoxib 12.5 mg for low back pain relief.
Harpagoside inhibits COX-2, iNOS, and TNF-alpha expression in vitro, supporting its traditional use as an anti-inflammatory agent.
