Kava (Piper methysticum) is a Pacific Island plant whose root contains kavalactones with clinically demonstrated anxiolytic effects. It is one of the most well-studied herbal anxiolytics but carries important safety warnings regarding hepatotoxicity.
Kava (Piper methysticum) has been used ceremonially and medicinally in Pacific Island cultures for over 3,000 years. The root contains six major kavalactones — kavain, dihydrokavain, methysticin, dihydromethysticin, yangonin, and desmethoxyyangonin — that modulate GABA-A receptors, inhibit norepinephrine uptake, and interact with dopaminergic pathways. A Cochrane meta-analysis of 11 RCTs confirmed that kava significantly reduces anxiety compared to placebo, with efficacy comparable to buspirone and certain benzodiazepines. Unlike benzodiazepines, kava does not appear to impair cognitive performance at standard doses. However, case reports of severe hepatotoxicity — including liver failure requiring transplantation — led to regulatory restrictions in several countries. The risk appears higher with acetonic or ethanolic extracts, products using aerial parts or bark peelings rather than noble-cultivar root, and concurrent alcohol use. Water-based extracts from noble cultivars of the peeled root (as used traditionally) appear to carry lower hepatotoxic risk. Current guidelines recommend limiting use to 12 weeks, using only standardized aqueous root extracts, and avoiding concurrent alcohol or hepatotoxic medications.
A Cochrane meta-analysis of 11 randomized controlled trials found kava extract significantly superior to placebo for reducing anxiety, with a standardized mean difference comparable to conventional anxiolytics.
Unlike benzodiazepines, clinical studies show kava produces anxiolysis without significant sedation or impairment of cognitive function at therapeutic doses.
Several trials have reported improved sleep quality as a secondary outcome of kava supplementation, likely related to its anxiolytic and muscle-relaxant properties.

Use only standardized aqueous extracts from noble kava cultivar roots. Limit use to 12 weeks maximum. Do not combine with alcohol or hepatotoxic drugs. Monitor liver function with prolonged use.
