Milk thistle (Silybum marianum) is an herbal remedy best known for its hepatoprotective properties. Its active complex, silymarin, has antioxidant, anti-inflammatory, and liver-regenerating effects supported by clinical evidence.
Milk thistle is a flowering plant in the Asteraceae family native to the Mediterranean region. Its seeds contain silymarin, a standardised extract comprising silibinin (silybin A and B), silychristin, and silydianin. Silymarin acts as a potent antioxidant by scavenging free radicals and increasing glutathione levels in hepatocytes. It stabilises hepatocyte membranes, inhibits the entry of toxins, and stimulates ribosomal RNA polymerase to promote liver cell regeneration. Clinical trials have demonstrated benefit in alcoholic liver disease, non-alcoholic fatty liver disease (NAFLD), and drug-induced liver injury. The European Medicines Agency (EMA) and WHO recognise milk thistle as a supportive treatment for liver conditions. Silymarin has low oral bioavailability (20–50 %), which has driven the development of phytosome complexes that significantly improve absorption. It is generally well tolerated with a favourable safety profile.
Silymarin stabilises hepatocyte membranes, increases glutathione synthesis, and stimulates liver cell regeneration. Clinical trials show improvements in liver enzyme levels in patients with alcoholic and non-alcoholic liver disease.
Silymarin scavenges free radicals, inhibits lipid peroxidation, and boosts endogenous antioxidant enzymes including superoxide dismutase and glutathione peroxidase.
Several clinical trials suggest that silymarin supplementation reduces fasting blood glucose and HbA1c in patients with type 2 diabetes, possibly through anti-inflammatory and antioxidant mechanisms in the liver and pancreas.
Divided into 2–3 doses, taken with meals
Higher doses (600–800 mg/day) used in clinical trials for liver disease. Phytosome formulations may allow lower effective doses due to improved absorption.
