Peppermint oil contains menthol, a potent antispasmodic that relaxes smooth muscle in the gastrointestinal tract. It has strong clinical evidence for relieving irritable bowel syndrome symptoms and tension headaches.
Peppermint oil is derived from Mentha piperita and contains 30–55% menthol and 14–32% menthone as primary active constituents. Menthol acts as a calcium channel antagonist on intestinal smooth muscle, producing direct antispasmodic effects that reduce colonic motility and visceral pain. Multiple meta-analyses of randomized controlled trials consistently demonstrate that enteric-coated peppermint oil is superior to placebo for global improvement of IBS symptoms, including abdominal pain, bloating, and altered bowel habits. The number needed to treat (NNT) is approximately 3, making it one of the most effective IBS treatments available. Topically, menthol activates TRPM8 cold receptors and inhibits TRPA1 pain receptors, providing analgesic effects for tension headaches when applied to the forehead, comparable to acetaminophen 1000 mg. Peppermint oil also exhibits antimicrobial and carminative properties. Enteric-coated formulations are essential for GI use to prevent esophageal reflux.
A comprehensive meta-analysis of 12 RCTs found enteric-coated peppermint oil significantly superior to placebo for global IBS symptom improvement (NNT = 3) and abdominal pain reduction. Benefits include reduced bloating, gas, and urgency.
Topical application of 10% peppermint oil to the forehead and temples reduced tension headache intensity as effectively as 1000 mg acetaminophen in a randomized, double-blind, crossover study.
