Also known as: Monascus purpureus rice, Hong Qu, Ang-kak
Red yeast rice is a fermented rice product containing monacolin K, a compound chemically identical to the prescription statin lovastatin. It has demonstrated significant cholesterol-lowering effects in clinical trials but carries the same potential risks as statin medications, including muscle toxicity and liver injury.
Red yeast rice (RYR) is produced by fermenting white rice with the mold Monascus purpureus, yielding a product that has been used in traditional Chinese medicine and cuisine for centuries. The primary bioactive component is monacolin K, which is chemically identical to lovastatin, a prescription HMG-CoA reductase inhibitor (statin). Meta-analyses of randomized controlled trials have demonstrated that red yeast rice supplements containing standardized monacolin K content significantly reduce total cholesterol, LDL cholesterol, and triglycerides. A landmark Chinese trial (CCSPS) involving nearly 5,000 patients showed that a standardized red yeast rice extract (Xuezhikang) reduced recurrent cardiovascular events by 45% over 4.5 years. However, because monacolin K is pharmacologically identical to lovastatin, red yeast rice carries the same risk of adverse effects, including myopathy, rhabdomyolysis, and hepatotoxicity. The monacolin K content of commercial products varies widely and is often poorly standardized. In the EU, supplements containing ≥3 mg monacolin K per daily dose have been restricted due to safety concerns. Red yeast rice should be treated as a pharmaceutical-grade intervention requiring medical monitoring, not a simple dietary supplement.
Meta-analyses demonstrate that red yeast rice containing monacolin K reduces LDL cholesterol by approximately 15–25% and total cholesterol by 10–20%, comparable to low-dose lovastatin therapy.
The Chinese Coronary Secondary Prevention Study (CCSPS), involving 4,870 post-MI patients, demonstrated a 45% reduction in recurrent cardiovascular events and a 33% reduction in all-cause mortality over 4.5 years with standardized RYR extract.
With the evening meal, similar to statin dosing
IMPORTANT: Monacolin K is identical to lovastatin. Treat red yeast rice as a statin medication. Requires medical supervision, liver function monitoring, and CoQ10 co-supplementation. EU regulations restrict products with ≥3 mg monacolin K per daily dose.

Some statin-intolerant patients tolerate red yeast rice better than prescription statins, possibly due to lower monacolin K doses or the presence of other beneficial fermentation compounds. However, the same statin-related risks apply.
