Thunder God Vine is a traditional Chinese medicinal plant with potent immunosuppressive and anti-inflammatory properties. While some clinical trials suggest efficacy for rheumatoid arthritis, it carries serious toxicity risks affecting the liver, kidneys, reproductive system, and bone marrow.
Thunder God Vine (Tripterygium wilfordii Hook F) is a perennial vine from the Celastraceae family, used for centuries in traditional Chinese medicine for inflammatory and autoimmune conditions. Its principal bioactive compounds include triptolide, celastrol, and tripdiolide, which possess potent immunosuppressive, anti-inflammatory, and antitumor properties. Triptolide inhibits NF-kB activation, suppresses T-cell proliferation, and reduces pro-inflammatory cytokine production. A notable head-to-head randomized trial published in the Annals of Internal Medicine found that a Tripterygium wilfordii extract was non-inferior to methotrexate for ACR50 response in rheumatoid arthritis, and the combination was superior to either alone. However, Thunder God Vine carries a narrow therapeutic window with severe dose-dependent toxicity. Serious adverse effects include hepatotoxicity, nephrotoxicity, bone marrow suppression, and male and female infertility (amenorrhea and azoospermia). Cases of fatal poisoning from improper preparation have been reported. Only the peeled root and root bark are used medicinally; the leaves, flowers, and outer root bark are highly toxic. This supplement should only be used under close medical supervision.
A randomized controlled trial found that Tripterygium wilfordii extract achieved ACR50 response rates comparable to methotrexate at 24 weeks, with the combination therapy outperforming both monotherapies for rheumatoid arthritis.
Triptolide and celastrol suppress multiple inflammatory pathways including NF-kB, T-cell activation, and pro-inflammatory cytokine cascades. These effects are pharmacologically potent but carry substantial toxicity risk.
