Also known as: Cobalamin, Methylcobalamin
Vitamin B12 is an essential water-soluble vitamin required for red blood cell formation, neurological function, and DNA synthesis. Deficiency is common in older adults and those following plant-based diets.
Vitamin B12 (cobalamin) is the largest and most structurally complex vitamin, containing the trace element cobalt. It serves as a cofactor for two critical enzymes: methionine synthase (which regenerates methionine from homocysteine and requires folate) and methylmalonyl-CoA mutase (essential for energy metabolism from fats and proteins). B12 is exclusively synthesized by microorganisms and enters the human food chain primarily through animal products. Absorption requires intrinsic factor, a glycoprotein secreted by parietal cells in the stomach — a process that declines with age and is absent in pernicious anemia. Deficiency causes megaloblastic anemia and can lead to irreversible neurological damage including peripheral neuropathy and cognitive decline if untreated.
B12 is essential for normal erythropoiesis. Deficiency leads to megaloblastic anemia characterized by large, immature red blood cells that cannot function properly.
B12 is required for myelin synthesis and maintenance. Adequate levels protect against peripheral neuropathy, cognitive decline, and mood disturbances.
B12 and folate are co-substrates in the methionine synthase reaction. Combined supplementation effectively lowers elevated homocysteine, a cardiovascular risk marker.
Morning, with or without food
Oral doses above 500 µg rely on passive diffusion (approximately 1% absorption), bypassing intrinsic factor dependence.
Sublingual or oral, any time of day
Plant-based diets provide virtually no bioavailable B12. Supplementation is essential.
Direct utilization without hepatic conversion
Cost-effective maintenance supplementation
